You can use this form to request a reprint of a registration certificate where:

Select All clients and click ASIC on the top menu bar. The Customer lodgements page appears.
Click the Agent documents button on the secondary navigation menu. The Agent documents lodgement page appears.
Click the Agent forms button at the top-right of the page.
Under Agent reports and actions, click Prepare form next to Certificate reprint.
Click on the drop-down arrow and choose the company you're requesting a reprint for.
Click I hereby declare that this information is true and that I am authorised to give this notification to ASIC.
Click Lodge form.
